Please fill out this new patient intake form and fax or mail it back to us at least a few days prior to your initial consultation. This will allow your naturopathic physician to go over the form and focus in on the key areas during your visit.

Your initial consultation will be 1 h 15 minutes. If there is a need, your naturopathic physician may choose to extend the visit by a few minutes to fully understand you as a whole person. Therefore, please allow ample time for your naturopathic visit and be courteous to us and others by arriving for your appointment on time.

If, for any reason, your appointment must be cancelled, please give us at least 24 hours notice. This allows us to serve other patient’s needs in your place.
